    Anal paps. I know, what the hell is that?!


    Well, it’s something you should at least be aware of.


    Anal cancers are relatively rare in general public (approximately 1-2/100,000), but there are some estimates that this becomes 40/100,000 for HIV- men who have sex with men (MSM) and an astounding 80/100,000 for HIV+ MSM. Anal cancer is something everyone should be concerned about. Among men who top, the risk of anal cancer is associated with 10 or more sexual partners and a history of anal warts, syphilis, or hepatitis. So, if you’re thinking “hey, this doesn’t affect me, I don’t EVER bottom!”…think again.


    Anal cancers are caused by the human papilloma virus, or HPV. Yes, this is the same virus that is implicated in cervical cancer and genital warts. Within a few years of sexual activity, many people have been exposed to HPV. Many people have been vaccinated against many strains of HPV, and the vaccine is close to being 80% effective. At this point, the HPV vaccine (Gardisil) is given to all males ~11-26 as well as women.


    So, do you need an anal pap? Well, maybe. While the rationale for anal pap testing is strong, routine testing for all MSM is not yet recommended by the powers that be- being the CDC, the U.S. Preventive Services Task Force and the American Cancer Society. Why? There isn’t enough existing data to make a firm recommendation. So, essentially, it is subject to change and will be a moving target in the coming years. Now, don’t get me wrong, many people need an anal pap right now; for example, here in NYS, they are performed for HIV+ men annually after a baseline is obtained (HIV makes HPV worse an HPV makes HIV worse!). Also, if a patient were to develop vague rectal symptoms that otherwise couldn’t be explained by other common disease processes, an anal pap would be recommended.


    The take-home message: talk to your doctor! Review your risk of common illnesses and STDs. Get vaccinated against HPV. Prevention is both a push and a pull. Protect yourself!

    This does not sound like a big deal. Tell me though why I have had two colonoscopies and have been WIDE AWAKE through both of them. Watching it all on the screen and commenting. And it does hurt when it goes around the bends and all. Why won't they listen when I tell them I have a high tolerance for sedatives and painkillers? They say 'we'll just give you extra' but it did not work either time.

    Also why was the Doctor so damn rude? It was not my fault you did not listen and give me the sedatives I needed so don't bitch when I am awake and asking questions-if you did your job you would not have to listen to me.

    Well, wow. That's...yikes. Sorry that this happened. Versed is usually pretty good-fast on and off and is sometimes used during these types of procedures. That's odd, I wonder if you're a super-metabolizer of benzos. If you're having surgery in the future, you should tell the anesthesiologist about this. It'll change how they treat you. Conscious sedation is not really supposed to be that THAT conscious.

    I had two friends (one male, one female) pass away from colorectal cancer. Get tested.

    I had a good friend pass away as well. It may feel good while your doing it, but it's still an ass and you must take care of it as best you can. We've all seen things that we know don't look good and healthy. Take some time and do the right thing.

    I'm sorry to hear that @LeoBlack22 and @SwagJack .

    Prevention and screening can sometimes feel daunting.

    Gardasil (an HPV vaccine) is indicated up to 26 years of age for MSM. You can still get it after this age, though it can be pricey- $200 a pop for a series of 3 shots. Yes, expensive, but likely worth it. I paid out of pocket for it since I missed my deadline.

    Now, regarding anal paps? There are still no real guidelines. However, there are some sources that say HIV +ve MSM should have anal paps annually and HIV -ve MSM over 40 y/o should have anal paps every 2-3 years. The crucial thing though- talk to your doctor-they will help you make the right decision.
